Health care visits can be overwhelming. Here are resources to help you prepare for your first visit.
Welcome to the LSS Health Center!
Patient Forms
When possible, we ask that you print and complete the below new patient registration forms and bring them to your first appointment. Registration forms are available in several languages, and we also offer interpretation services if you do not see your preferred language listed below.
Along with your completed registration forms, please also bring:
- Photo ID if available
- Documents to determine sliding scale enrollment
- Insurance card if available
- Current medication list
- Current pharmacy name and location if available
- Previous medical history if available
If you would prefer to have your medical records sent to LSS Health Center ahead of your scheduled appointment, please have them faxed to LSS Health Center at 855-208-4527.
Location
LSS Health Center is located at 245 N Grant Ave. Please enter using the main building entrance and the Health Center will be on your right. There is ample on-street metered parking but limited parking lot spaces.
Services are provided Monday – Friday 8 a.m. – 5 p.m.
LSS Health Center is also accessible by bus. Please view COTA’s website to find the best route.
Interpretation Services
If you require interpretive services, please let us know prior to your appointment and we can arrange for an interpreter. LSS Health Center provides translation services through Language Line and Access2Interpreters, with translation services available for over 200 languages.
