- Are or have been homeless in the past few years
- Are “doubled up” living with friends or family
- Are in supportive or transitional housing
- Have difficulty finding care
Patient Privacy
Our care coordinators meet with every new patient and established patients as needed to assess for social determinants of health and assist with needs, including same-day Medicaid through Presumptive Eligibility, Medicaid Applications, SNAP applications and other services.
We take your privacy seriously. All patients of LSS Health Center are provided access to our Notice of Privacy Practices, and you may also read the full notice here.
