Our assessment specialist (an RN and your doctor) can help you or your loved one determine home care needs. Evaluations can usually be performed within 24-48 hours at no cost. We are happy to make recommendations for durable medical equipment and assist in obtaining equipment and other services as necessary.

Many times, your doctor will contact us through a referral or a hospital doctor will refer you. After the initial assessment (which includes a complete head to toe assessment, blood pressure, pulse, respirations and temperature, discussing what you can do at home, discussing your home care needs, etc.), the RN will present the information to your doctor. The doctor and nurse will discuss your needs. The nurse will follow your doctor’s orders. The number of visits may be daily or monthly based on your specific needs.

LSS Home Health Care serves individuals in Columbus, Ohio and throughout central Ohio, including the following counties:

  • Franklin
  • Delaware
  • Fairfield
  • Morrow
  • Ashland
  • Richland

Our office hours are Monday through Friday 8:00 a.m.-5:00 p.m. We have someone on call/available during hours the office is closed. An RN is also available.

Yes. An RN will manage the care you need with your doctor. We also have a director of nursing on staff.

Yes, if there are any changes in your condition and as needed.

Your doctor and the RN who manages your care.

There may be several reasons to terminate care. The most common is you have progressed to a level that your doctor and nurse agree you are able to care for yourself without our assistance.

Yes, we contract with a reputable company that serves our clients with registered physical therapy, occupational therapy and speech therapists.

Yes. Our billing specialists will work with your insurance company to determine the level of coverage available to you.

No, your needs are determined by your doctor, which may be a doctor who took care of you in the hospital. Your primary care physician will take over your care usually some time after discharge.
